[cabf_validation] Validation Method Revisions - Ballot 169

Here are the  changes I received from Jeremy based on our discussions during
the  F2F in Bilbao.  Plus I inserted the  footnote on examples of Request
Tokens previously omitted per Question 2 below.

I created a branch, but then realized that  Peter already had one, so I made
a change to his version -  see

Question 1 - this doesn't include any changes made at the  face-to-face,
were there any?

 

Question 2 - I noticed that  the examples of  Request Tokens (at the bottom
of the document "Domain Validation WG Proposal") aren't in this draft.
Shouldn't they be?
